We’re growing fast and are looking for an experienced, people‑focused Team Lead to guide and support a team of Care Coordinators delivering high‑quality, person‑centred care management services. In this role, you’ll provide leadership, performance oversight, and operational support to ensure services align with organisational priorities and aged‑care regulatory requirements, while fostering a collaborative, high‑performing team culture. What You’ll Do - Lead, coach and develop team capability through regular performance discussions and development conversations. - Monitor individual and team performance against KPIs, service standards and operational targets. - Drive operational improvement by developing action plans, refining policies and workflows, and supporting the delivery of organisational strategy and service goals. - Provide strong line‑management leadership, including leave management, attendance oversight and workload allocation. - Lead high‑quality telephone‑based care coordination, supporting care planning, reviews and care plan updates. - Support care recipients to navigate services under the Support at Home program and achieve person‑centred outcomes. - Act as an escalation point for complex care matters, escalating to management where required. What You’ll Bring - Minimum 2 years’ leadership experience in a human services setting (e.g. aged care, disability, NDIS, or health support) - Demonstrated success in team development, performance management, and service delivery - Exceptional verbal and written communication skills, with a client‑centred approach. - Strong interpersonal skills with the ability to mentor, coach, and engage team members effectively - Proven problem‑solving and conflict resolution capabilities - Confident using Microsoft 365, CRMs, and digital care platforms in a fast‑paced, high‑volume, remote‑first environment - Experience in aged care, community services, healthcare, or care coordination environments. (Desirable) - Knowledge of the Support at Home program and aged care legislation.
